I have brother who was transferred here a week but unfortunately he passed away on October 7th due to their lack of care. They never checked on him. My family and I are very devastated and heartbroken. My aunt was thinking of looking for medical records of why my brother passed away. Please take your loved one somewhere better than here. We may have to sue them‼️

I do want to compliment a young lady by the name of Elsa in your social worker team. My sister was recently released from Affinity, and I requested the assistance of Elsa in a matter that pertained to contributing to my sister's health and well being. She's a busy lady so initially, it took a moment to connect. However, once we did and expressed to her how she could contribute to the well being of my sister following her release from your facility, Elsa went above and beyond the call of duty to assist our family. I don't want to go into depth regarding the efforts that Elsa made, but they were abundant and yield excellent results. I feel that the contribution Elsa provided in this matter will directly impact not just my sister's health, but also our entire family. I must add that she did not necessarily have to do any of the additional help that she provided since it may have been outside of the realm that constituted her doing her job. However, she showed a human, caring side; she made it her duty to assist without expectations, and she did so not only during working hours, but also followed up with us outside of her shift. Please, give Elsa a pat on the back (preferably a raise!) since she would not accept lunch from me as a token of our gratitude for her actions. I hope more of your employees follow suit, and I definitely know that more patients will benefit from her being part of your staff.

The MOST RECENT 5 STAR OR HIGH REVIEWS are from the new management and family and friends. For the past two years this facility’s review has been 2.5 stars. All of a sudden during the acquisition , it went up to 4.4!? Is this not suspicious to anyone?!
How sad and dishonest at the same time !!!!!!

Please save yourself stress and time. Our patient got multiple UTI and candida due to not enough staff to take her to bathroom. The attending dr Bishey/ Kabbany never calls family back. You call the place and the hold is at least 15 minutes and then you get cut off!!!!! So so sad for all the patients there with no advocate.
It’s so interesting that only high reviews get a feedback. Lol.

Wish everyone who has a loved one here best OF LUCK! Unless you are their advocate, the well-being of the patients declines on a daily basis.

This facility used to be same old "
Affinity Health care" with 2 star reviews. They changed name , but residents still get neglected. Stay away if you love your family.

They keep getting fined due to neglegence. Residents wear each other clothes even thought the articles of clothing has been written the residents name on it. Just a below average place!!!!!keep looking.

Do yourself a favor and look at the reviews on " Affinity Health Care Center". It's a 1 Star. The name just changed. Stay away if you love your loved ones.

My mom was here because she was in recovery. However I feel like they failed us! My mom grew a bed sore on her back that was so deep you could see her bone and it was so big. They claimed they were turning her but honestly I feel like they only did that when I was there. They failed us. My mom is no longer with me. I hate this place.

Lobby looks nice. Cucumber infused ice water. Sometimes the place smells when my team and I drop off a new pt for admission. Charge RN/Staff not very friendly and only wants minimal report. Staff on their phones in the hallway even when call lights are ringing. They use wrist bp devices which are not the most accurate. They dont turn pts enough. This place is on my list of gross and bad facilities.

I was here for rehabilitation for 10 days at the end of May/beginning of June. I was very concerned due to the poor reviews. I have to say that I was very pleasantly surprised. I had a very nice room, facing the garden patio. It was very clean, floors washed every single day, walls were scrubbed whenever scuffs from carts, wheelchairs, etc. appeared. I was treated very well. Staff was very knowledgable & efficient. If I met someone only once, they remembered my name & always greeted me by name in the hallway......I was impressed. The entire staff worked very hard to keep patients as comfortable as possible. Never met a group of harder working people than the staff at this facility. They all knew their job & what was expected of them. Never heard any arguing among the staff, they all pitched in to get the job done. This was my first experience being a patient in a nursing facility, however I have visited many & this was, by far, the very best.
