My wife, Joan,.has been cared for over the last 10 months at Brookdale. During this time the few incidents , falls, have been handled in a professional manner. The staff is excellent and show strong interest in the residents.

I feel the choice of Brookdale to care for Joan was a good one.

Great event today! If you’re looking for a memory care community in Citrus Heights, give them a call. Jerilyn, Executive Director, China, and Sarah all work very hard to ensure residents are well cared for and have a sense of purpose every day!

When my family made the very difficult decision to put my father in a memory care facility, we fell in love with Brookdale immediately. The grounds were beautiful, there was a large safe area for dad to walk in, he's a mover so this was important. The staff was friendly and attentive, and we clicked instantly with Tracy, Jerilyn and China.
We moved Dad in on 10/13/23 at 11:50 am, the first few weeks were rough, understandably, there were medications to adjust and routines to establish but by the end of the 3rd week we knew there was something going on that was not right. On 11/11/23 we gave our 30-day notice simply saying that it wasn't a right fit, praying that the treatment wouldn't get worse. We removed him from the home on 12/15/23, my mother having spent a quarter of her life savings for 65 days in that home. In those 65 days, my father was abused and neglected by the people we trusted with his life.
The entire time dad was at Brookdale the home was short staffed, we were told this by numerous people on different occasions. The nurses were not happy about it and talked amongst themselves about it frequently. Mid November I listened to nurses and a MedTech discussing people having urinated on themselves and how disgusting that was. Nurses talking about how to deal with the “troublemakers” by rounding them up early and making them sit at the table long before breakfast or lunch were ready. A nurse (the same one each time, female, white, stocky build, and short hair) frequently found sitting on a chair in a hallway between wings to watch two groups of people (all she was watching was her cell phone). MedTech (female, African American medium build shoulder length black hair) complained about being a care giver because of short staffing instead of just dispensing meds. Lashawna, (Lay for short) complained of having too many residents and needing them to “slow down”. This short staffing coincided with my father’s door being locked on at least 7 separate occasions that we know of. When his door was locked, he had NO ACCESS TO A BATHROOM!!
We would find dad bloodied from a fall that NO ONE knew he had taken. No fluids available to him, no bathroom available unless he knew where his room was (he did not), and his room was constantly locked. Every single time we visited with dad he needed two things immediately, water or milk and a bathroom. EVERY. SINGLE. TIME. They kept him locked out of his room so he wouldn't rearrange it, and they withheld fluids to prevent him needing to urinate. They stuck him in diapers, my once proud, amazing father, in diapers! HE WAS NOT INCONTINENT! HE WAS BEING LOCKED OUT OF THE ONLY BATHROOM HE HAD ACCESS TO.
Toward the end of November and beginning of December, my father was so lethargic from dehydration he was difficult to understand. He was a man on the move his whole life and that made him more difficult to handle than the older folks who sat and watched TV all day, for that, the nurses and MedTech's abused him. The change in my dad from the day we brought him there to the day he left was the most heartbreaking thing I have ever seen. This was our first time with outside care as my mother had handled it previously herself. Brookdale robbed my mother of her ability to trust a home or outside care. They made everything so much worse.
In the end, I sent all of my concerns and complaints to the director of the home, Jerilyn Purol, at first, she seemed attentive and concerned. I gave her copious notes on what happened and when it happened and who was involved etc. She email back and forth and asked questions, I really felt like maybe the staff was the problem and management didn't know.
I'm a fool, she just stopped responding to me one day. Please be careful if you decide to visit Brookdale, what you see is not what you get and it'll cost you your loved ones safety, their dignity and your life savings.

My Mom has been in 4 homes so far and Brookdale Sylvan Ranch is the BEST! The staff is attentive, caring, insightful and coordinate with the family. The last place she was at had OK staff, but nothing like the quality here. I can't say how much I appreciate their patience and efforts to keep everyone safe and entertained. I highly recommend this location for your loved one living with any form of dementia. Mom is safe, well fed, cared for! Thank-you!

I wanted to add on to my review. Mom passed in June after a couple of hard falls. The staff was wonderful to work with and very caring. We all had a couple of good cries over losing Mom. We all didn't want to lose her this way, but that is the path of life sadly. I stayed with her, played her recordings from the family, called all my siblings so they could talk with her, and let her choose when she would leave us. When I could not be with her the staff was with her, they cleaned and changed her, they gave her comfort and pain medications. Green Valley Hospice worked closely with the Brookdale staff to ensure that Mom had a peaceful passing. I miss my friends I made there, I miss visiting you all, but I know life moves on. I can't recommend your services enough to everyone looking for a loving home for a loved one. Thank-you!

My dad was in a couple of different places for assisted living as well as some Skilled Nursing Facilities, although this was the only dementia care facility. That said, I was very impressed with the facility and service he received. More than anything, I think you want a place where the staff are kind and attentive and the director and management are attentive. Brookdale Sylvan Ranch earn high marks on both accounts. Issues occasionally popped up (as will happen with dementia patients with health issues), but I was always impressed with the willingness of on-site management to work with us (and my Father). The staff were also top notch. I could see my dad really took to them and was comfortable around them. I also never say a staff member ever get frustrated, which I was impressed with. Unfortunately my Father recently passed away from complications of some disabilities he had. However the staff there made a difficult time as easy as it could have been. I was very thankful for that. The grounds and layout are also very accommodating for residents and the single rooms were wonderful. If I had any gripe it would be a little more turnover in management that I would like, but its a minor gripe. Overall, I would definitely recommend this facility.
